  • Already started
  • Operating Mon and Tues currently
  • Located in the GPAU

GP IN A&E

  • Benefits -Appropriate GP Care in ED setting -Effective discharge communication -Opportunistic care -Reduction in Admissions

GP EXTENDED HOURS

  • Aim to commence early autumn
  • Will be based in the City of Coventry Health Centre
  • Likely to offer appointments 6-10pm weekdays and 8-2pm weekends
  • Main access routes will be the GP practice and 111

GP Extended Hours

  • Benefits -Routine and Emergency Doctor & Nurse appointments OOH -Relieves pressure on GP Practices, can book directly into Hub -Clinical Systems linked so routine QOF can be done benefitting both practices and patients

Frailty Unit

  • MDT coordination of discharge of complex elderly pts
  • 12-28 bed ward based in UHCW with Gerontology input

  • Record sharing with both read and write functionalitiy
  • Sharing of care plans
  • Practices will have the ability to 'accept' consultaions
  • IG framework is currently being established and we will seek the input of the LMC
  • Integration with current systems

  • Medicine management and prescription hub
  • Help with 'back room' functions, eg Payroll, returns,
  • Training for practice staff especially reception, HCAs, admin
  • Help with CQC documents
