Professional & Polite

• Virtual survey
• Surveyor on iPad carried by QM nurse or representative
• iPad held to maintain privacy
• Side conversations can be heard! Keep conversations quiet and away from survey team
• When invited by surveyor, answer what you know, offer to find out if you don't know
• PPE and social distancing

C4 Care environment

Unit/Hall/Rooms/Storage

• Clear clutter
• No food/drink at nurses station
• Linen carts covered (nothing on top of them)
• Nothing within 18" of sprinklers
• no cardboard shipping/packing boxes on the unit

Med room

• Clean, no meds out, no trash on counters
• Most current references posted (look alike/sound alike)
• fridge clean, meds labled, dated, no food

Nurse documentation

• audit: assessment, care plans complete
• fall, pain, skin, v/s complete
• restraint documentation
• Patient education
• Be able to explain pt hand off (everyone)
• Code cart checks up to day (clean that up)
• All QC logs completed
• Do not use abbreviations; orders signed/dated
