INTERIOR EVENTS
Engine 2 connected 100ft. of 2.5in. to standpipe
E2 flowed water for several minutes before exiting due to low air
Rescue 3 headed up North stairwell and got on original hose line
Other crews had brought up additional high-rise packs
When additional packs were charged, multiple kinks created

INTERIOR EVENTS CONT.
R3 reported no water on hose line
Two of R3's crew were low on air so crew moved to stairwell
The two FF's exited but the captain (victim) continued down hallway
Other two FF's went after victim, afterward all three were low on air
Victim was confused and they headed back to stairwell

INTERIOR EVENTS CONT.
One FF got separated but found exit at window with ladder
Victim ran out of air and asked partner to buddy breath
Victims disconnected regulator as parter attached buddy breather
All of the air then escaped victims face piece
Victim then finally transmitted Mayday and activated PASS

FF also attempted to activate PASS and transmitted multiple Maydays
FF searched and found door to South stairwell and placed axe in door
FF returned and retrieved victim who had vomited in mask and removed it
FF collapsed at top of stairwell and both fell down the stairwell
FF were found due to victims PASS device in the third floor stairwell

KEY RECOMENDATIONS
Ensure existing SOP's for high-rise ops are reviewed & enforced
Ensure a deployment strategy for high-risk incidents is developed
Ensure the IC develops an IAC, which is communicated to all FF's
Ensure the IC utilizes division/group supervisors
Ensure that FF's are properly trained in air management

Ensure FF's are properly trained in out-of-air SCBA emergencies
Ensure that IC is provided a chief's aide to assist in incident management
Ensure that IC establishes a stationary command post for incident management
Ensure that FF's are properly trained in Mayday SOP's and survival techniques
