Basically, the Soviets were in a huge rush to launch this R-16 rocket before Nov. 7 to commemorate the anniversary of the revolution, so they were under a lot of pressure and cut corners in testing, etc. Even when problems occurred on launch day they ignored many safety procedures, did work on the rocket while it was fueled when normally they would remove the fuel before working on it, and ignored small problems that cropped up with the rocket components. On top of that, apparently one of the circuits was routed directly to the second stage engine from the ground accidentally. And because of the rush and the problems, the engineers were still swarming all over the rocket when they should have been out of the area only a half hour before launch time. Apparently what happened, and you should read the book to verify I have all this correct, was an order was given to set a certain switch to a "ready" point a half hour before liftoff, when it wasn't supposed to be set to that until a minute before launch, possibly to save time. The faulty circuit allowed the impulse to go directly to the second stage, igniting the second stage engines which caused the ensuing explosion of the first stage as well, killing some 100 engineers and scientists. But definitely read the sections on the disaster to get a better understanding of it.
