Undersized baby's usually have underdeveloped bodies. Their lungs may not be able to work on there own and they may have to spend there first few days or even weeks attached to a respirator. Once the baby can breath on its own the baby may continue to have problems with there lungs.

How it affects the baby's heart.
Babies whose mother smoked are up to 70% likely to have a heart defect at birth, and there heart is more than likely not to function normally. And there heart usually pumps blood slower.
