A piece of foam insulation broke of the external tank and damaged the left wing
As the shuttle re entered the atmosphere, the damage caused atmospheric gases to break down the internal wing, causing the shuttle to slowly break down
Killed all seven crew members

The o-ring from the solid rocket booster broke at liftoff, causing a breach in the SRB joint it sealed, allowing pressurised hot gas from within the solid rocket motor to reach the outside. This led to a structural failure of the external tank, causing the entire shuttle to disintegrate.
Killed all seven crew members, all happened within 78 seconds
