New Vaccine Protocol

Current Vaccine Protocol Problems

  • Not being addressed on every patient at admission
  • Patients are discharging without vaccination status being addressed
  • **Not meeting CMS core measures for vaccination standards
  • The new process is simple, it should not interupt your current workflow

Step One - Admitting RN

  • Vaccination status is a required question of the admission process in the EHR
  • Over-ride, or skipping, of questions has been disabled to avoid fall-outs
  • Admission RN will have patient sign consents/refusals of unnecessary vaccine(s)
  • RN will fax consents to pharmacy for vaccine orders

Step 2 - Vaccine(s) Needed

  • An alert for each vaccine due will be generated by the EHR
  • The alert will remain in the E-MAR as a medication due until it is administered
  • Admission or Staff RN to administer vaccine(s)
  • Once administered, the alert will disappear but vaccination(s) will remain as part of the patient record to be transcribed into the discharge summary

Step 2 - Vaccine(s) not Needed

  • Admitting RN will click vaccine not needed
  • The reason will be documented for each refusal
  • Information will be automatically transcribed into the discharge summary
  • **Please ensure refusals are signed by RN and patient then placed in the chart

Step 3 - Discharging RN

  • Verify signed consents/refusals for vaccines and make a copy for the patient
  • If vaccine was ordered and not administered, administer prior to discharge
  • Educate patient on importance of maintaining up-to-date vaccination status
  • Print discharge summary as well as educate patient on patient portal access.
