Person Centered Planning

A Strength-Based Approach to Supporting People with Disabilities

What is Person Centered Planning?

  • An ongoing problem-solving process used to help people with disabilities plan for their future
  • A focus on an individual and their vision of what they would like to do in the future

Why Person-Centered Planning?

  • To focus on a person's strengths and look at a person in a different way
  • To increase opportunities for positive participation in the community
  • To recognize individual desires, interests and dreams
  • Through team effort, develop a plan to turn dreams into a reality

Who is Involved?

  • Most important: The person who the meeting is about!
  • Anyone who loves and cares about the individual, including parents, caregivers, therapists, friends, neighbors, etc.
  • A facilitator to guide the discussion

First Steps - Getting to know you

  • Develop a history or personal life story of the focus person
  • Learn about the personal preferences of the focus person, including what they do and don't enjoy doing
  • Collect various perspectives from all team members, including the individual

Next steps: Making a Plan

  • Share visions for the future
  • Identify obstacles and opportunities
  • Identify Strategies
  • Identify action steps and implement them

Helpful Hints to ensure success

  • All participants should have a clear & shared appreciation of the talents, capacities, desires of the focus person
  • The group involved agree to meet regularly to review progress
  • The group includes a strong advocate for the individual, and a person committed to implementing the plan.
