PerFORM ASSESSMENT

We assess our patients to develop a customized plan based on their medical history, chief complaints, signs and symptoms.

TrEatment Goals

The key goal of an appropriate care plan is to treat the patient's condition in the best way possible. Important steps are to:
1. Identify the problem
2. Determine cause of problem
3. Develop strategy for dealing with problem
4. Implement that strategy
5. Restore the patient's condition to as close to normal as possible. When it is not possible to restore to normal conditions, acceptable conditions must be listed as the target for care.

Care Plans may serve as a guide for individual therapies, such as aerosol therapy, broncho-pulmonary hygiene, oxygen therapy, hyperinflation techniques, suctioning, and pulse oximetry.

Once all data is compiled and a treatment plan is reached the respiratory care practitioner then calls the patient's physician to deliver the recommendations and receive a confirmation of the respiratory order.
