The clinical inquiry proposed will be: Does implementing a one page visual aid to symptoms management of heart failure in addition to usual discharge teaching improve readmission outcomes?
My practicum experience has enlightened my aim on the need to provide patients and families with practical and meaningful information as well as resources they can utilize within the natural environment of their home to assist them in self-care management.
Effective self-care, with daily symptom monitoring and knowledgeable decision making about symptoms when they occur, may help patients with HF maintain an acceptable health related quality of life (HRQOL) and avoid repetitive hospitalizations.
The Theory of Symptoms Management will provide the framework for a care approach to prevent and control acute exacerbations of HF. The Self-Care of Chronic Illness Theory will provide a structure of self-care that addresses the process of maintaining health within the context of management required for the chronic disease of heart failure. The Theory of Relationship Based Care will support the significance of an ongoing relationship with the patient and family as vital in sustaining self-care.
Clinical implications and next pragmatic steps refer to this projects' transferability to other health disciplines and chronic illnesses for use in transitional care strategies.
Future goals of this tool will hope to include the development of an IPhone application for the patient to report health status changes guided by the HF symptom management zones.
