The story depicted in this painting is the vision Saint John the Evangelist has of the virgin mother Saint Anne, who conceived the Virgin Mary. The story is from the book of Revelation - chapter 12, verses 1-4. Saint John saw the woman with the 'moon under her feet' and crying as if she was about to give birth.

"A great sign appeared in heaven; a woman clothed with the sun, with the moon under her feet and a crown of 12 stars on her head. She was pregnant and cried out in pain as she was about to give birth"
REVELATION 12:1-4

The painting is one of his earliest creations, painted in 1618-19. It was made to hang in a convent in Seville alongside the painting 'Saint John on the Island of Patmos'
