Three Mile Island opened September 2, 1974.
The name was based off the plant being located three miles downriver from Middletown, Pennsylvania.
The accident occurred March 28, 1979

In simple terms, the feed pump stopped operating. The turbine water was unable to remove heat from the steam generator . In result, the control rods dropped into the reactor, which interrupted the fission process. Temperatures rose, pressures dropped. The reactor core began to melt, and the hot zirconium reacted with the water; this produced hydrogen gas, which is combustible.

No one died in result of the accident itself. Although, people suffered from being exposed to radiation and died years later.
